The challenge
AGL’s Loy Yang A Power Station is a cornerstone of Victoria’s electricity supply. The Unit 2 Major Outage was a high-consequence event delivered under tight timeframes, complex interfaces, and a large transient workforce, where quality failures could directly impact safety, schedule, and return-to-service reliability. Key risks included rework, weld integrity, foreign material ingress, documentation completeness, and effective change management within a live, asset-critical operating environment.

The Impact Plus solution
Engaged by Service Stream, a Tier-1 Contractor, Impact Plus led quality planning, quality management, and quality reporting for the outage. Our approach was risk-based and informed by lessons learned from the previous Unit 4 Major Outage, deliberately shifting assurance upstream rather than relying on reactive inspection. We aligned the Service Stream Quality Management Plan with AGL’s requirements and embedded quality expectations early through planning, workforce inductions, procurement decisions, and competency verification. Throughout execution, Impact Plus provided structured quality governance — including inspection and test planning, audit oversight, IMTE control, welder qualification verification, FME assurance, and close-out reporting — supported by real-time insights that enabled timely decision-making and early intervention before return-to-service was impacted.

The Outcome
Strong quality performance, low rework, and clear improvement pathways.
The Unit 2 Major Outage achieved strong quality outcomes, including a low weld failure rate of 1.3%, no major cut-out and re-weld failures, and full calibration compliance for all inspection and test equipment.
Over 670 Inspection and Test Plans were generated and managed, and more than 580 personnel completed quality-focused inductions, reinforcing consistent expectations across the workforce.
Importantly, the structured quality framework enabled transparent identification of improvement opportunities, particularly in change management, digital system readiness, and QA resourcing. These insights now provide a clear pathway for continuous improvement across subsequent major outages at Loy Yang A.
